An "advance health care directive" lets your physician, family and friends know your health care preferences, including the types of special treatment you want or don't want at the end of life, your desire for diagnostic testing, surgical procedures, cardiopulmonary resuscitation and organ donation.

Advance Health Care Directive Form
The Advance Health Care Directive Form from the State of California is available online from the State of California and from various health facilities like Kaiser Permanente Medical, San Jose, in the Santa Teresa area.
The San Francisco Department of Health has a very clear online presentation of this form.
The form must be signed by two witnesses or one notary public. If you are in a nursing home the nursing home patient advocate or ombudsman must be one of the witnesses.
